A web app for collecting and displaying data about accidents. It allows the customer's partners to create and customize surveys about accidents to their needs. Surveys could have complex structures with dynamic fields that were shown or hidden depending on the input of other fields. Members of the partner could fill in the survey when an accident occurred. The app also included an admin panel to manage partners and their surveys. Valuable accident statistics were the result of the data collected.
